Object: Diffuse midline glioma (DMG) is a highly aggressive and lethal brain tumor predominantly affecting children and young adults. Previously known as diffuse intrinsic pontine glioma (DIPG) or grade IV brain stem glioma, DMG has recently been reclassified as "diffuse midline glioma" according to the WHO CNS5 nomenclature, expanding the DMG demographic. Limited therapeutic options result in a poor prognosis, despite advances in diagnosis and treatment. Radiotherapy has historically been the primary treatment modality to improve patient survival. Methods: This systematic literature review aims to comprehensively compile information on the diagnosis and treatment of DMG from 1 January 2012 to 31 July 2023. The review followed the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) statement and utilized databases such as PubMed, Cochrane Library, and SciELO. Results: Currently, molecular classification of DMG plays an increasingly vital role in determining prognosis and treatment options. Emerging therapeutic avenues, including immunomodulatory agents, anti-GD2 CAR T-cell and anti-GD2 CAR-NK therapies, techniques to increase blood-brain barrier permeability, isocitrate dehydrogenase inhibitors, oncolytic and peptide vaccines, are being explored based on the tumor's molecular composition. However, more clinical trials are required to establish solid guidelines for toxicity, dosage, and efficacy. Conclusions: The identification of the H3K27 genetic mutation has led to the reclassification of certain midline tumors, expanding the DMG demographic. The field of DMG research continues to evolve, with encouraging findings that underscore the importance of highly specific and tailored therapeutic strategies to achieve therapeutic success.

BACKGROUND: Preclinical and clinical evidence implies that destructive therapies in local and malignant tissue are frequently used on patients with head and neck cancer. Consequently, the microbiome of the treated and adjacent regions is affected. Disruption of the normal microbiome plays an important role not only in the disease progression but also in its emergence, therefore new therapies involving probiotics, prebiotics, and synbiotics have been developed to control or regulate this microbial disruption. OBJECTIVE: This review aims to describe the current and potential uses of probiotics at different stages of development of head and neck squamous cell carcinoma, as an adjuvant therapy to prevent common complications such as radiation-induced oral mucositis (RIOM) and its role in other areas. METHODS: Currently, there is no widely effective strategy to treat or prevent this kind of cancer. Surgery, radiation therapy, and chemotherapy are the three main treatments for head and neck cancer. Some therapies can also cause long-term health problems, or complications which might change the way you eat, talk, hear and breathe. RESULTS: The main uses for which probiotics have been studied are: Prevention and reduction of severity of RIOM, change in dental plaque to reduce dysbiosis, and reduction of complications in post-operated patients. Potential uses of probiotics include the reduction of disease initiation and progression by reducing local inflammation caused by bacteria and other organisms. CONCLUSION: The incidence and severity of RIOM may be lessened by probiotics. To establish its uses in additional clinical settings, though, more studies are necessary.

RESUMEN El riesgo de cáncer de ovario a lo largo de la vida de una mujer es menor de 2 %, y durante la vida reproductiva es de 0,01 %. La incidencia de cáncer de ovario durante el embarazo es de 1: 10 000 - 100 000 casos. La mayoría de las masas anexiales en la gestación son asintomáticas. El diagnóstico se realiza frecuentemente mediante un hallazgo casual en la ecografía obstétrica; se ha estimado una incidencia de 0,2 - 2 %, y de estas, 1 - 6 % son malignas. El carcinoma epidermoide desarrollado en un teratoma quístico maduro es una rara neoplasia que representa, aproximadamente, el 2 % de los tumores primarios del ovario. Se presenta una paciente con diagnóstico accidental de teratoma ovárico maduro con diferenciación de carcinoma epidermoide, durante la realización de una cesárea.


ABSTRACT The risk of ovarian cancer throughout a woman's life is less than 2%, and during reproductive life it is 0.01%. The incidence of ovarian cancer during pregnancy is 1: 10,000 - 100,000 cases. Most adnexal masses in pregnancy are asymptomatic. Diagnosis is frequently made by casual finding on obstetric ultrasound; an incidence of 0.2 - 2% has been estimated, and 1 - 6% of them are malignant. Squamous cell carcinoma developed in a mature cystic teratoma is a rare neoplasm that represents approximately 2% of primary ovarian tumors. We present a patient with an accidental diagnosis of mature ovarian teratoma with differentiation of squamous cell carcinoma, during a cesarean section.

RESUMEN Fundamento: el cáncer es una de las principales causas de morbilidad y la primera causa de mortalidad en el mundo, entre el 30 % y 40 % de los pacientes diagnosticados se curan; el resto, inevitablemente transitará hacia etapas avanzadas y terminales, necesitará de cuidados paliativos. Objetivo: diseñar un curso de superación profesional para médicos de familia, a través del cual adquieran los conocimientos y habilidades requeridos para brindar cuidados paliativos al paciente con cáncer. Métodos: se realizó una investigación basada en el método dialéctico-materialista, con enfoque cualitativo, en el Policlínico Universitario "XX Aniversario", entre septiembre 2019-marzo 2020. Se emplearon métodos teóricos: análisis-síntesis e inducción-deducción; empíricos: análisis documental sobre contenidos relacionados con cuidados paliativos en pregrado y posgrado de la carrera de Medicina y cuestionario a los médicos para la autovaloración de sus carencias; y los matemático-estadísticos para los valores absolutos y relativos. Resultados: los profesionales muestreados autoevaluaron como regular sus conocimientos generales sobre cuidados paliativos al paciente con cáncer, la mayoría no se sintieron capacitados para brindarlos, mientras otros reconocieron insuficientes los cursos de superación, por lo que se diseñó uno compuesto por nueve temas, con sus objetivos, contenidos y formas evaluativas, el cual fue valorado por criterios de especialistas. Conclusiones: el curso de capacitación diseñado fue valorado como muy adecuado en sus aspectos: pertinencia, asequibilidad y tratamiento científico-metodológico de sus temáticas.


ABSTRACT Background: cancer is one of the main causes of morbidity and the first cause of mortality in the world, between 30% and 40% of diagnosed patients are cured; the rest will inevitably go through advanced and terminal stages, they will need palliative care. Objective: to design a professional improvement course for General Practitioners, through which they acquire the knowledge and skills required to provide palliative care to cancer patients. Methods: an investigation based on the dialectical-materialist method, with a qualitative approach, was carried out at the "XX Aniversario" University Polyclinic, from September 2019 to March 2020. Theoretical methods were used: analysis-synthesis and induction-deduction; empirical ones: documentary analysis of contents related to palliative care in undergraduate and postgraduate studies of Medicine and a questionnaire for doctors to self-assess their deficiencies; and the mathematical-statistical ones for the absolute and relative values. Results: the professionals sampled self-evaluated as average their general knowledge about palliative care to patients with cancer, most did not feel qualified to offer them, while others recognized insufficient improvement courses, so a course composed of nine topics was designed, with their objectives, contents and assessment forms, which was valued by specialist criteria. Conclusions: the designed training course was valued as very adequate in its aspects: relevance, affordability and scientific-methodological treatment of its topics.

BACKGROUND: Premature infants contribute to infant morbidity and mortality especially in low resource settings. Information on tocolytic and/or anti-inflammatory effects of several plant extracts, such as citral, could help prevent preterm birth cases and reduce the number of preterm infants. The aim of this study was to evaluate the in vitro tocolytic and anti-inflammatory effect of citral on myometrial tissues of the human uterus. METHODS: Myometrial samples from uteri obtained after hysterectomy were used in functional tests to evaluate the inhibitory effect of citral on PGF-2α induced contractions. The intracellular cyclic adenosine monophosphate (cAMP) levels generated in response to citral in human myometrial homogenates were measured by ELISAs. Forskolin was used as a positive control. The anti-inflammatory effect of citral was determined through the measurement of two pro-inflammatory cytokines, tumor necrosis factor-α (TNFα) and interleukin (IL)-1ß, and the anti-inflammatory cytokine IL-10, in human myometrial explants stimulated with lipopolysaccharide (LPS). RESULTS: Citral was able to induce a significant inhibition of PGF-2α induced contractions at the highest concentration level (p < .05). Citral caused a concentration-dependent increase in myometrial cAMP levels (p < .05) and a concentration-dependent decrease in LPS-induced TNFα and IL-1ß production, while IL-10 production increased significantly (p < .05). The anti-inflammatory and tocolytic effects induced by citral could be associated with an increase in cAMP levels in human myometrial samples. CONCLUSION: These properties place citral as a potentially safe and effective adjuvant agent in preterm birth cases, an obstetric and gynecological problem that requires urgent attention.

INTRODUCTION: The COVID-19 pandemic has changed health systems across the world, both in general hospitals and in oncology institutes or centres.For cancer specialists, particularly breast cancer (BC), the COVID-19 pandemic represents a combination of challenges since the hospital resources and staff have become more limited; this has obliged oncology specialists to seek a consensus and establish which patients with BC require more urgent attention and which patients can wait until there is a better control of this pandemic. The health system in Latin America has some special characteristics; in some of the countries, there are shortages which limit access to several specialities (surgery, clinical oncology and radiotherapy) in some regions. OBJECTIVE: After a systematic review of the most recent literature regarding the management of BC during the COVID-19 pandemic, the main objective is to understand the position of the different Latin American Societies of Mastology in terms of available alternatives for the treatment of BC. METHODS: After carrying out a comprehensive and exhaustive search of the most recent guides on the management of BC during the COVID-19 pandemic, the board members of the Latin American Federation of Mastology invited, via email, different specialists, all experts in BC care, to complete an anonymous survey online.The survey was distributed between 30 and 10 May 2020. The survey included 27 questions on four topics: demographic information, consultations, imaging and treatment of BC.The questionnaire was sent and then distributed to various health specialists including breast surgeons, clinical oncologists, radiation oncologists and radiologists via the Presidents of the different Latin American Societies of Mastology in 18 countries. The results are summarised as tallies based on the number of responses to each question. RESULTS: A total of 499 responses were received. The majority of the respondents were males (275 (55.11%)); 290 participants were over 45 years (58.11%).The questionnaire presented those surveyed with three possible answers (agree, disagree and neither agree nor disagree). The results reflect that there was consensus in the majority of situations presented. Only seven questions revealed disagreement among those responding. The results are presented as recommendations. CONCLUSION: The management of patients with BC presents unique challenges during the current world health situation produced by COVID-19 pandemic. Breast care specialists (surgical oncologists, breast care clinicians, clinical oncologists, radiation oncologists and radiologists) from 18 countries in Central and South America submitted through their responses and recommendations for the treatment of BC during the COVID-19 pandemic.

Despite the wide application of carvacrol (CAR) in different biological and medical areas, there is still insufficient electrophysiological data on the mechanisms of action of CAR, particularly in the pregnant uterine function. The aim of this study was to evaluate the in vitro tocolytic effect of CAR on the contractility of isolated pregnant rat uterus in the presence of a calcium channel antagonist (nifedipine) and a cyclooxygenase inhibitor (indomethacin). The uteri were isolated from pregnant Wistar rats at 16-18 days of pregnancy and suspended in an isolated organ bath chamber containing a Ringer's physiological solution and aerated with 95% O2and 5% CO2. Samples were used in functional tests to evaluate the inhibitory effect of CAR at increasing concentrations on the rhythmic spontaneous, oxytocin-induced phasic, K+-induced tonic, and Ca2+-induced contractions. The differences in inhibitory concentration-50 and Emaxamong the compounds were determined using the one-way ANOVA followed by a post hoc Student-Newman-Keuls or Bonferroni test, in all casesP < 0.05 was considered statistically significant. Nifedipine was used as positive controls where required. CAR caused a significant concentration-dependent inhibition of the uterine contractions induced by the pharmaco- and electro-mechanic stimuli. We showed that the inhibitory effects of CAR depends on the type of muscle contraction stimuli, and that it acts stronger in spontaneous rhythmic activity and in contractions of isolated rat uterus induced by Ca2+. Nifedipine was more potent than CAR and indomethacin on the uterine contractility (P < 0.05), but none of them was more effective than nifedipine. Therefore, the tocolytic effect induced by CAR was associated with the blockade of the calcium channels in the pregnant rat uterus. This property placed CAR as a potentially safe and effective adjuvant agent in cases of preterm labor, an area of pharmacological treatment that requires urgent improvement.

Seed dispersal is an essential process to maintain the viability of plant populations, and understanding this ecological process allows management strategies to be developed to conserve ecosystems. European Union priority habitat 5220* is defined as "Mediterranean arborescent shrubland with Ziziphus lotus" and it represents a favorable microclimate within the severe climatic conditions typical of the semiarid south-eastern region of the Iberian Peninsula. Therefore, the study of seed dispersal in this priority habitat by different frugivorous guilds, is a challenge for its conservation. In this study, we have characterized a mutualistic network of seed dispersal that is mediated by vertebrates (mammals and birds) in the protected habitat 5220*. The aims of this study were to: (i) identify the seed disperser community; (ii) analyze the relative role of key species in the dispersal process; and (iii) compare the functional ecology of the seed dispersal process between mammals and birds. As such, we collected animal faeces to determine seed dispersers taxonomy, identifying the mammals through the visual aspect of the faeces and the birds by DNA barcoding. In the case of birds, we also collected regurgitated seeds in which the disperser species was also identified through molecular techniques. This allowed us to build-up a mutualistic network and to identify the relative role of these animals in seed dispersal. Our results showed that mammals and birds fulfilled complementary roles in seed dispersal, with birds representing the main dispersers of key plants within the 5220* habitat, and mammals the main dispersers of human-cultivated plants. Herein, we provide a useful approach with relevant information that can be used to propose management policies that focus on restoring the threatened 5220* habitat, promoting the role of birds to disperse key species that structure plant communities of this priority habitat.

The liver is considered the laboratory of the human body because of its many metabolic processes. It accomplishes diverse activities as a mixed gland and is in continuous cross-talk with the endocrine system. Not only do hormones from the gastrointestinal tract that participate in digestion regulate the liver functions, but the sex hormones also exert a strong influence on this sexually dimorphic organ, via their receptors expressed in liver, in both health and disease. Besides, the liver modifies the actions of sex hormones through their metabolism and transport proteins. Given the anatomical position and physiological importance of liver, this organ is evidenced as an immune vigilante that mediates the systemic immune response, and, in turn, the immune system regulates the hepatic functions. Such feedback is performed by cytokines. Pro-inflammatory and anti-inflammatory cytokines are strongly involved in hepatic homeostasis and in pathological states; indeed, female sex hormones, oral contraceptives, and phytoestrogens have immunomodulatory effects in the liver and the whole organism. To analyze the complex and interesting beneficial or deleterious effects of these drugs by their immunomodulatory actions in the liver can provide the basis for either their pharmacological use in therapeutic treatments or to avoid their intake in some diseases.

Introducción: El linfoma extranodal de células NK/T, tipo nasal es una variedad agresiva que se presenta con mayor frecuencia en Asia Oriental y América Latina. El régimen de tratamiento SMILE, basado en dexametasona, metotrexato, ifosfamida, L-asparaginasa y etopósido, mostró tasas de respuestas objetivas adecuadas en estudios de fase 1/2. Objetivo: presentar el primer reporte de la aplicación del protocolo SMILE en Cuba. Presentación del caso: paciente de 47 años de edad, blanca que presentó recaída temprana de linfoma extranodal de células NK/T tipo nasal en partes blandas de la pared anterior izquierda del tórax. Recibió quimioterapia SMILE por 4 ciclos y en la evaluación al final del tratamiento se comprobó que hubo una respuesta completa de la enfermedad(AU)


Introduction: Extranodal lymphoma of NK / T cells, nasal type is an aggressive variety that occurs most frequently in East Asia and Latin America. The SMILE treatment regimen, based on dexamethasone, methotrexate, ifosfamide, L-asparaginase and etoposide, showed adequate objective response rates in phase 1/2 studies. Objective: to present the first report of the application of the SMILE protocol in Cuba. Case presentation : 47-year-old white patient who presented early relapse of extranodal lymphoma of nasal NK / T cells in soft parts of the left anterior wall of the thorax. He received SMILE chemotherapy for 4 cycles and the evaluation at the end of the treatment showed that there was a complete response of the disease(AU)

Menkes disease is a neurodegenerative and lethal pathology caused by gene mutations of the copper-transporting ATP-7A enzyme; it manifests itself by neurological symptoms and connective tissue changes of varying severity. Timely subcutaneous use of copper histidinate (Cu-His) is determinant for quality of life. We report the first experiences in Mexico on Cu-His synthesis and its safe use in 3 cases where hypocupremia and hypoceruloplasminemia were corroborated. With advice of the Hospital for Sick Children of Toronto Canada, we prepared a 500 µg/mL solution. In all three cases were 250 µg of Cu-His applied without relevant undesirable effects for 30 days. Serum copper (Cu, expressed in µg/L) and ceruloplasmin (Cp, in mg/dL) were determined: case 1, Cu days 0 and 30, 8 and 504 µg/L; Cp days 0 and 30, 4 and 10.75 mg/dL; case 2, Cu days 0 and 30, <50 and 502 µg/L; Cp days 0 and 30, 2 and 15 mg/dL; case 3, Cu days 0 and 30, 3 and 84.2 µg/L; Cp days 0 and 30, 4 and 10.7 mg/dL. In Mexico, it is possible to safely synthesize Cu-His and treat MD, which must be intentionally sought.

With advice of the Hospital for Sick Children of Toronto Canada, we prepared a 500 µg/mL solution. In all three cases were 250 µg of Cu-His applied without relevant undesirable effects for 30 days. Serum copper (Cu, expressed in µg/L) and ceruloplasmin (Cp, in mg/dL) were determined: case 1, Cu days 0 and 30, 8 and 504 µg/L; Cp days 0 and 30, 4 and 10.75 mg/dL; case 2, Cu days 0 and 30, <50 and 502 µg/L; Cp days 0 and 30, 2 and 15 mg/dL; case 3, Cu days 0 and 30, 3 and 84.2 µg/L; Cp days 0 and 30, 4 and 10.7 mg/dL. In Mexico, it is possible to safely synthesize Cu-His and treat MD, which must be intentionally sought.

BACKGROUND: Colorectal cancer (CRC) is the second and third most frequent cancer in women and men, respectively; indeed, CRC is placed as the fourth world's most deadly cancer (after lung, liver, and stomach cancer). The incidence of CRC is strongly influenced by nutrition and the high fat/high carbohydrate Western-style diet. CRC is one of the most intensively studied cancer types, partly because of its high prevalence, but also because of the existence of its precursor lesions, tubular or villous adenomas, and more recently serrated adenomas. The morphological steps in the adenomacarcinoma sequence have been elucidated at a molecular level, which allow the identification of the genes responsible for CRC. Review and Conclusions: The main aim of this review is to provide data regarding the pathophysiological characteristics, molecular mechanisms as well as carcinogenic and chemopreventive agents for CRC, with emphasis on evidence supporting their efficacy. These compounds may modulate multiple signaling pathways involved in cell proliferation and apoptosis in transformed cells, they also enhance the host immune system and favor an effective treatment. Despite promising results from experimental studies, only a limited number of these compounds have been tested in clinical trials. The mechanistic spectrum and specificity of the action of phytochemicals represent a complex and evolving field of research.

The aim of this study was to evaluate the in vitro anti-inflammatory and utero-relaxant effect of α-bisabolol on the pregnant human myometrium. Samples from the pregnant human myometrium were used in functional tests to evaluate the inhibitory effect of α-bisabolol (560, 860, 1,200 and 1,860 µM) on spontaneous myometrial contractions. The intracellular cyclic adenosine monophosphate (cAMP) levels generated in response to α-bisabolol in human myometrial homogenates were measured by ELISA. The anti-inflammatory effect of α-bisabolol was determined through the measurement of two pro-inflammatory cytokines, tumor necrosis factor-α (TNFα) and interleukin (IL)-1ß, and the anti-inflammatory cytokine IL-10, in pregnant human myometrial explants stimulated with lipopolysaccharide (LPS). Forskolin was used as a positive control to evaluate the cAMP and cytokine levels. α-Bisabolol was found to induce a significant inhibition of spontaneous myometrial contractions at the highest concentration level (p<0.05). α-Bisabolol caused a concentration-dependent decrease in myometrial cAMP levels (p<0.05) and a concentration-dependent decrease in LPS-induced TNFα and IL-1ß production, while IL-10 production did not increase significantly (p>0.05). The anti-inflammatory and utero-relaxant effects induced by α-bisabolol were not associated with an increase in cAMP levels in pregnant human myometrial samples. These properties place α-bisabolol as a potentially safe and effective adjuvant agent in cases of preterm birth, an area of pharmacological treatment that requires urgent improvement.

El año 1994 marcó una línea temporal de avance y desarrollo en la investigación de cierto grupo de linfomas, caracterizados por la afectación, generalmente extranodal, en la línea media facial, la nasofaringe y la piel. La Organización Mundial de la Salud, y su más reciente revisión en el 2016, los agrupó dentro del apartado de neoplasias maduras de células T y natural killer como linfomas extraganglionares. Se presenta a una paciente, blanca, de 71 años de edad, con diagnóstico de un linfoma cutáneo de células T/natural killer tipo nasal, angiocéntrico en estadio avanzado. Se trata de un linfoma poco común, pero con curso clínico agresivo, con múltiples factores de mal pronóstico y escasa respuesta al tratamiento, que ocasionó el desenlace fatal de la paciente.

The aim of this study was to evaluate the relaxant and anti-inflammatory effects of two thalidomide analogs as phosphodiesterase-4 (PDE-4) inhibitors in pregnant rat uterus. Uteri from Wistar female rats were isolated at 19 day of pregnancy. Uterine samples were used in functional studies to evaluate the inhibitory effects of the thalidomide analogs, methyl 3-(4-nitrophthalimido)-3-(3,4-dimethoxyphenyl)-propanoate (4NO2PDPMe) and methyl 3-(4-aminophthalimido)-3-(3,4-dimethoxyphenyl)-propanoate (4APDPMe), on prostaglandin-F2α (PGF2α)-induced phasic, K+-induced tonic, and Ca2+-induced contractions. Accumulation of cAMP was quantified in uterine homogenates by ELISA. Anti-inflammatory effect was assessed by using ELISA for determination of the pro-inflammatory cytokines tumor necrosis factor-α (TNFα) and interleukin (IL)-1ß, and anti-inflammatory IL-10, from uterine explants stimulated with lipopolysaccharide (LPS). Nifedipine, forskolin and rolipram were used as positive controls where required. Both thalidomide analogs induced a significant inhibition of the uterine contractions induced by the pharmaco- and electro-mechanic stimuli. Nifedipine and forskolin were more potent than the analogs to inhibit the uterine contractility, but these were more potent than rolipram, and 4APDPMe was equieffective to nifedipine. Thalidomide analogs increased uterine cAMP-levels in a concentration-dependent manner. The LPS-induced TNFα and IL-1ß uterine secretion was diminished in a concentration-dependent fashion by both analogs, whereas IL-10 secretion was increased significantly. The thalidomide analogs induced utero-relaxant and anti-inflammatory effects, which were associated with the increased cAMP levels as PDE-4 inhibitors in the pregnant rat uterus. Such properties place these thalidomide analogs as potentially safe and effective tocolytic agents in a field that urgently needs improved pharmacological treatments, as in cases of preterm labor.

Se realizó una investigación descriptiva y transversal de 64 pacientes (muestra de tamaño calculado) con insuficiencia renal crónica, dependientes del tratamiento hemodialítico, y atendidos en el Servicio de Hemodiálisis del Hospital Provincial Docente Clinicoquirúrgico "Saturnino Lora Torres" de Santiago de Cuba, de mayo del 2013 a igual mes del 2014, con vistas a identificar la disfunción diastólica en ellos a través de sus características ecocardiográficas. En el estudio sobresalieron los pacientes del sexo masculino en las edades de 40 a 59 años y la disfunción diastólica de tipo II, con fracciones de acortamiento y de eyección y contractibilidad normales; asimismo predominó el remodelado cardíaco concéntrico (hipertrofia ventricular izquierda), principalmente en aquellos que habían recibido hemodiálisis por más de un año.


A descriptive and cross-sectional investigation of 64 patients (calculated size sample) with chronic kidney failure depending on hemodialytic treatment, and assisted at the Hemodialysis Service of "Saturnino Lora Torres" Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba, was carried out from May, 2013 to the same month of 2014, aimed at identifying the diastolic dysfunction in them through their echocardiographic characteristics. In the study the patients of the male sex aged 40 to 59 years and type II diastolic dysfunction were the most outstanding, with normal reduction, ejection and contractibility fractions; also the concentric heart remodeling prevailed (left ventricular hypertrophy), mainly in those undergoing hemodialysis for more than a year.
